feat: add bottom navigation shell with four tabs

Introduces AppShell with an IndexedStack-backed NavigationBar (Today, History, Insights, Settings). Stub screens added for Calendar, Insights, and Settings; app.dart now routes to AppShell instead of LogMoodScreen directly.
